Our care teams work to develop innovative care improvement strategies such as interdisciplinary care teams designed to partner with patients through their care journey, and strive to protect our vulnerable patients through eliminating healthcare-acquired conditions such as falls, pressure ulcers and infections while developing early warning systems that alert care teams to intervene rapidly with subtle changes in a patient’s condition indicating decline.

Commitment to Outcomes

Each year, our care communities are assessed by the states in which we operate to ensure we are in compliance with standards and provide a safe and healing environment. These surveys are standard in all skilled nursing facilities, and are a part of our licensure and help calculate our CMS 5-star rating.

We take these surveys very seriously and understand that the care we provide takes place all year long. For this reason, we have a dedicated team tasked with overseeing our clinical quality and ensuring we proactively measure quality and opportunities for improvement.

Commitment to Oversight

Each of our communities receives clinical oversight and direction by a Medical Director. In addition, nurse practitioners oversee care in some of our care communities with high-acuity patients. These clinicians provide an added level of expertise and direction. At Avalon, our Chief Medical Officer works to ensure each of these care professionals are up to date with the latest industry trends and efforts and helps recruit and retain medical professionals.
