Our Story
AVALON CARE CENTER AT NORTHPOINTE offers short-term placement for patients recovering from either an accident or injury requiring skilled nursing and/or skilled therapy care. We offer a 20 bed sub-acute unit with a committed team of nursing and physical, occupational and speech therapy professionals. We have a dedicated Medical Director and supporting medical staff that provides medical supervision and assessments at bedside. This dedication provides the patient with a continued focus on recovery and well-being while at Avalon Care Center. We provide the care and services necessary to help the patient make a successful transition home.
Our community features both private and semi-private accommodations help our patients heal with comfort. We have large resident spaces with wide open day rooms and large spacious areas. Our goal is to provide an environment with comfort and compassion to support continued care and goals of each patient and family. Avalon Care Center at Northpointe is a premier facility of choice by patients and providers alike for skilled nursing and rehabilitation services.
